Collinsville Unit 10 announces kindergarten enrollment schedule

Enrollment for 2014-2015 kindergarten students in Collinsville Unit 10 will be held at the Administration Annex Building, 123 West Clay Street, on the dates listed below for each school.

If you are not sure which school your child will be attending, please contact the district enrollment office at 346-6350 ext. 237 or 228.  All spots at each school will be on a first come first serve basis

Each parent enrolling their child on their school’s enrollment date will receive a special treat to take home for the kindergarten student.

The above enrollment schedule is for: children entering kindergarten next year, including any child previously enrolled in Pre-K or Early Childhood, children entering first grade who are not presently attending kindergarten classes in the public schools.

Children must be 5 years-old on or before Sept 1, 2014.

To enroll your child, you will need to provide a certified birth certificate (he certificate issued by the hospital is not acceptable according to Illinois State Law), Social Security Number of the child and proof of residency (see below).

On the first day of school, your child must present a completed physical examination form with all immunizations listed as required by Collinsville Unit 10.  Your child will also be required to have a dental and eye exam.

Please note that all grade levels must also register at the Centralized Registration this summer at Collinsville High School. Dates are July 22, 23, and 24. A make-up day is scheduled for July 28.  Registration is from 1-7 p.m. each day.

Establishing student residency

1. Must present a certified or registered birth certificate for the student at the time of enrollment along with their Social Security number

2. Category I: (One document required)

 Most recent tax bill and proof of payment e.g. a canceled check

 Mortgage papers (homeowners)

 Closing papers on a recently purchased house

 Signed and dated lease and proof of last month’s payment e.g. canceled check

AND

3. Category II: (One document showing proper address is required)

 Current driver’s license or state identification card

 Current vehicle registration

 Current public aid card with student’s name

AND

4. Category III: (One document showing proper address is required)

 Most recent (within 60 days) utility bill

For example: gas, electric, cable television, trash, water bill (No cell phones)

 Registration packet that has been mailed to your residence over the summer for the year 2014

Anyone with a Custody Order Seeking to Enroll a Student

Present court order, agreement, judgment or decree that awards or gives custody of the student to any person (including divorce decrees awarding custody to one or both parents).

Non-Parent Seeking to Enroll a Student

Must complete and sign Evidence of Non-Parent’s Custody, Control, and Responsibility of a Student form, along with district approval. (This must be done on a yearly basis).

Notes

The School District reserves the right to evaluate the evidence presented. Merely presenting the items listed does not guarantee admission.

If a student is determined to be a non-resident of the district for whom tuition must be charged, the persons enrolling the student are liable for non-resident tuition from the date the student began attending a district school as a non-resident.

A person who knowingly enrolls, or attempts to enroll, in the school district, on a tuition-free basis, a student known by that person to be a non-resident of the district is guilty of a Class C misdemeanor, except in very limited situations, as defined in State law (105 ILCS 5/10-20.12b(e).

A person who knowingly or willfully presents to the school district any false information regarding the residency of a student for the purpose of enabling that student to attend any school in that district without the payment of a non-resident tuition charge is guilty of a Class C misdemeanor (105 ILCS 5/10-20.12b(f).
